Kratom cases are exploding across the U S. and doctors are alarmed

Especially when combined with prescription medications, alcohol, and/or street drugs, kratom and 7-OH can cause severe respiratory depression and death. It also would regulate the product’s chemical content, mandate labeling with alkaloid amounts and warnings that kratom may be habit forming as well as bar kratom sales to anyone under 21. General physical side effects of kratom can include nausea, constipation, dizziness and dry mouth. According to the FDA, there were 11 deaths from 2011 to 2017 connected to kratom exposure, but almost all of them involved other drugs or contaminants. The drug has stimulant effects, in low doses, so people have reported feeling alert, physically energized and talkative, according to the Drug Enforcement Administration.

She argues 7-0H is being used by people looking to kick opioid addiction. The similarity to opioids is what Florida Attorney General James Uthmeier focused on when announcing the ban last week. “There is an activating component to opioids, and that’s what people talk about with kratom,” Perrone said. When kratom first started gaining traction in the U.S. two decades ago, its popularity was largely fueled by people trying to quit opioids. As with most herbal products, the research on drug interactions with kratom is incomplete. If the amounts of certain chemicals are higher or lower in different products, this can increase or decrease the effects of kratom. Herbal products, such as kratom, naturally contain many different chemicals.

“I tried to put out some public education alerts and nobody was interested,” said Stanciu, an assistant professor of psychiatry at the Geisel School of Medicine at Dartmouth. “So, you know, to be able to use an opioid use disorder treatment on somebody with 7-OH use really solidifies for me — eyes open — that this is really the same problem.” “Kratom was one problem, but that was a lower dose of exposure of the compound that binds to the mu-opioid receptor,” Perrone said. But addiction specialists and researchers say that 7-OH is a different, more potent and much more dangerous beast. The patient did have a history of opioid use disorder, but he was in recovery. “It looked really like fulminant opioid withdrawal,” she said, “had all the components of opioid withdrawal.”
